Request to Review Child Support Form Assistant

This Form Assistant will help you complete the Job and Family Services (JFS) administrative request to review or modify a child support order (Form JFS 01849). It typically takes 15 to 20 minutes to complete.

This is not a court filing. This form starts the administrative process with JFS to review and possibly modify your existing child support order. If your support order is modified, the payment amount for any arrears may also change.

This interview can only be used to request a review or modification for one child support order at a time. If you have more than one order, you’ll need to complete the interview separately for each one.

Once you have downloaded the completed form, you will need to submit it to the county Child Support Enforcement Agency (CSEA) listed at the top of the form. You may:

  • Submit it online through the Ohio Child Support Customer Service Portal.
  • Print and Mail it to the address shown. Include all required documentation. 
  • Print and Hand-deliver it. Include all required documentation. 

Follow these steps to submit your form online: 

  • Log into the Customer Service Web Portal or Mobile App.
  • If you do not have an account, click “Creating an Account.” Follow the steps for creating your account. You will need to have your driver’s license or State identification number, and your social security number. 
  • After creating your account, find the Message Center in the Menu Bar.
  • From Message Center, select “New Message.”
  • Under Message Topic, select “Modify My Order.”
  • Select the Case/Order you are asking to be modified.
  • Click the “Attach Support Documents” icon to upload any attachments (this includes the completed and signed application for review and any supporting documentation).
  • Add any additional details in the message section.
  • When complete, click “Send.”

You can attach documents up to 10MB in size. If your documents are larger than that, you can send multiple messages and attach part of the support documents to each one.

Requesting an administrative review of your child support order may result in changes to:

  • Your monthly child support amount
  • Cash medical support
  • Repayment of arrears
  • Medical support provisions

These amounts may increase, decrease, or remain the same based on the outcome of the review.

Within 15 days of receiving your request, the CSEA will determine whether a review will take place. You cannot withdraw your request on or after the scheduled review date.

If a review is scheduled, the CSEA will mail a notice to you, the other parent, and any third-party caretaker. This notice will go to the mailing address you provided in this interview. The notice will also ask you to provide financial information. This may include filling out a financial affidavit, sharing details about medical support, and giving any other information needed to review the child support order.
