BMV Transfer on Death Form

Use this form to set up transfer on death for cars and other motor vehicles in Ohio. This will let you transfer your car to your loved ones without a will and avoid probate court.

If you're married, your spouse can transfer an unlimited number of vehicles, worth up to $65,000, to themselves after you die using the BMV's surviving spouse form without this TOD.
