Annual Guardianship of the Person Report

This interview will help you complete the forms to file the annual report for guardianship of the person of an adult in Ohio. This interview does not provide any forms or information for Guardianship of the Estate or Guardianship of Minors.

Review the forms to make sure that they are complete and accurate. Then:

Step 1. Save and print your forms. 

The forms will need to be printed single-sided, but don't sign anything yet. If you don't have a printer, you may be able to print your forms for a low fee at the local public library.

Form 17.1 – Statement of Expert Evaluation MUST be completed by a medical doctor, a clinical psychologist, an independent social worker, or a professional clinical counselor who is licensed in the state of Ohio; or an Individual Disability Team. The evaluation or examination shall be completed within three months before the date of the report. The prospective ward’s provider may be used to completing this form and may complete a version of this form that they already have on file. If they go to the doctor in a county that is different from the county where you are filing your application, make sure that they complete the uniform version of this form that you downloaded or emailed from Ohio Legal Help or is included in your packet. Your court might not accept a version of the form that is from a different county.

Step 2. Make photocopies. 

After you have signed make a copy for your records.

Step 3. File Take the forms and copies to the Clerk’s office at the Probate Court. 

Do not include this cover page with the forms you submit to the Clerk's office. Clerk staff can help make sure everything is complete, but will not be able to help you fill anything out.
