School Letter: Special Education Evaluation

Welcome to the Special Education Evaluation Form Assistant

This interview will help you write a letter to your child's principal requesting an evaluation for special education services. This is the first step in the process to create an Individualized Education Program (IEP) for your child. Learn more about when a child may qualify for an IEP and the full process here. 

This interview will take about 10 minutes. At the end of the interview you can download or email a .pdf version of the letter with instructions for next steps.
